The DES, according to the authors, "is a brief, self-report measure of the frequency of dissociative experiences." The scale was developed to provide a reliable, valid, and convenient way to quantify dissociative experiences.

This screening tool can help determine whether you might have the symptoms of Dissociative Identity Disorder (DID). Use the results to help decide if you need to see a doctor or other mental health professional to further discuss diagnosis and treatment of DID.

Instructions: This questionnaire consists of twenty-eight questions about experiences that you may have in your daily life. We are interested in how often you have these experiences. It is important, however, that your answers show how often these experiences happen to you when you are not under the influence of alcohol or drugs. To answer the questions, please determine to what degree the experience described in the question applies to you and click the number to show what percentage of the time you have the experience.
