"How Do I Explain My Social Phobia To A Date?"

I'm 20 years old and when I was 14, I was diagnosed with anxiety disorders and social phobia. I just started feeling like I want to date, but I don't know how to explain my problem to guys. I still get very antsy in public places and I still have panic attacks from time-to-time. How would I explain this to a guy without scaring him off. Has anyone else ever had this problem? I'm afraid I'm going to be alone the rest of my life.
